    Are both conditions required for this issue or is it a case of one or the other?

    Hi Ashish!

    This error message usually appears when the JIRA app is first installed or if it is re-installed, however, if you are running into this issue on your app that's been in use for a while, it would still be a good idea to run through the disconnect/uninstall workflow described in the resolution steps in the article.
