Why are tickets created via Facebook showing the requester as Facebook User instead of their name?

Answer 

This happens when the end user disables the option to interact with Apps, Websites and Games both on and off Facebook within their Facebook settings. Disabling this Facebook setting prevents their personal data, such as their name, from being shared.
