Submit a Request - no input fields showing

New to Zendesk, but not to developing... customers in our new Help Center click on "Submit a Request" to get a form with no input fields except "Attachments (optional)". Doesn't look like JS issue. Same on Chrome (latest) and Safari(latest). Are we missing a setting somewhere? Help!

Hey Chris,
Have you actually built out the ticket fields for your form? Might be best to make sure that the fields are not set to all be agent-only fields. If they are agent only fields then those fields will not appear on the form.

Hey Dawn,
Most likely there's some custom code in your Guide theme that is causing this issue. I would recommend testing this by previewing the default Copenhagen theme as mentioned in the following article: Previewing your help center theme
You may want to get your developers involved to see if they can assist with this further as well.
