Customizing Help Center Forms
I have searched for an article that answers this plainly but have not been able to do so. Is it possible to either:
1) Add the Name field to the Submit A Request form without having to create a custom field?
OR
2) Add a subject line field to the help widget so the description doesn't end up in the subject field for these submissions?

In order to give your users a place to add their name in your Submit a Request form you'll need to add a Custom Ticket Field. Once you've done that, you'd also have the option to add that custom field to your Web Widget - you can add up to 2 custom fields in the Widget setup.
As for the Subject, at this time there's no way to customize the Widget to include a Subject field and prevent the description from going into the Subject field of the ticket that's generated.
However, you do have the ability to change the subject of the ticket after it's been created, to remove the description text and add something short and descriptive.

I really need a subject line field in my web widget too!
Can zendesk please look to add this soon? As having the entire long description in the subject line is just messy in lists and the auto emails sent out to our clients.

Hey. I have the web widget and its contact form.
But how do I get into a contact form or the contact form into my help center.
Do I embed a code?

You'll need to get the Javascript code from the Widget settings page and then embed it. I'd recommend checking out [this article](https://support.zendesk.com/hc/en-us/articles/203908456#topic_pp3_pgd_bq) for more information.

On the Team plan, is it possible to edit the CSS for the submit a request contact form?

That is determined by your Guide plan - you would need to be on Guide Professional in order to edit the CSS for that form.

step 1.Go to Widget -> contact us enable it.
step 2. Go to ticket fields and create fields to appear in the form.
step3. Go to Ticket form, click on contact us. drag and drop the fields you require
