    Could you provide some screenshots on what you see from your end? Also, could you describe the desired result you are trying to achieve compared to what is currently taking place?

    This sounds like your form may be feeding in from the Support SDK for Android.  I would suspect that there is something coded in your app that is setting the subject line and not showing that field.  It may be worth checking out the developer docs on the Support SDK to see if there is any code that may be contributing to this behavior. 

    withRequestSubject seems to allow you to set a subject line, so that might be a place to start searching in the code.
