Limit custom ticket field access to just admins
I'd like to have a custom field on tickets where admins / team leads can flag the ticket to be excluded from an agent's CSAT metrics. I can't do this because we can't currently limit field visibility / edit access to just admins.
Would be good to have more granular control over custom field security.

Have you checked out the Field Manager V2 app in the marketplace? It should allow you do to exactly that. We're currently using it for preventing write access to certain custom fields on a ticket

Overall, it worked. One limitation is that the settings are global, so you can't set one field to be exclude one group from read-only, and another field to exclude a different group. For now it works fine, so thanks for the recommendation.
For Zendesk product team, this is still well short of equivalent functionality in SFDC.

Haven't tested this, so it might not work, but you might be able to install multiple copies of the app and restrict each app to a specific group. Doing so would let you come up with group-specific configurations.
Totally agreed that a native solution is needed, but in the interim, if you can have multiple installs of the same app, you might have a path forward.
