    I don't know if this would help you. We have checkboxes on some of our forms that relate to a custom field. When the box is unchecked the related field(s) don't show.  In the form conditions we have it configured that when the box is checked, the other field comes into view.  In my example below, when Add New User is checked the appropriate fields appear, otherwise they aren't there.

    Looks good, I will use this model in various different scenarios.

    For in topic mentioned specific situation, notes need to be seen all the time, for example, to write in notes, what else needs to be done, before checkmark can be added. And checkmark would add a tag, what further are involved in triggers, to proceed actions related to each checkbox.

    Right now, that it's like more problem with the design, would be good enough even if "title" for notes field wouldn't be required. So notes field would be under, checkbox field.

    Hey both! I've achieved this with conditional formatting on the ticket Form.

    So for example, if you wanted the three fields in your screenshot to show up when you click "Add New User", you simply add those as conditional based on the response to the checkbox field.

    We have a form which gathers several screens of information and it was making the Form too long, so we hid several of the fields under this "Show All Screens" checkbox, and here is a screenshot of the Conditions set on this Form.
