any suggestion/sample code  on below requirement

Requirement : - (End –User Window only)
“Last Name”, “First Name”, “Network ID”, “Reports To” and “Location” fields needs be pre populated and read-only for new requests
When “Is this request for someone else?” Field is checked and User Type field is selected as ‘Network Users’ and Email Address filed is entered
