Thank you Brandon Tidd This worked but I had one question for you. I noticed that when i set this trigger to fire based on the condition that the field in question was present it fired 33 times in a few seconds. Am I missing something to only make it fire once? 

This feature eliminated the platforms ability to add internal notes as a trigger action. This makes zero sense and has complicated our workflow , and taken away some of the value we had in this platform.
