Customize Talk Widget fields to make mandatory

We would like to make "Name" and "How can we help" mandatory fields in the Request a Callback form. Will save our agents time and we need to get this info anyway.  Thanks!

This is an absolute necessity for this feature. We would also like to collect emails too. But, at minimum, getting a name and summary of the call makes things much more efficient.
