I've set this up, but I'm finding 2 or 3 responses later in the conversation that 'pausable update' is suddenly triggered again when the ticket was not put in pending in the last few back and forth...

This feature is very important to us as well, and the lack of SLA on internal notes has caused a lot of frustration. I really hope it gets put on the product roadmap soon.
