    There is an option that will allow you to do this, but it requires custom code in your help center theme.  You can take a look and see if this article would help in your scenario: Can I add a disclaimer to a specific ticket form in Help Center?

    It's possible you are asking about inside of the agent interface as well, this would be a cool feature.  While it's not specific to ticket forms, you can do some interesting things with making certain forms, macros, and apps available using Contextual Workspaces based on the contents of a ticket.  It might be a way to start making more relevant information more easily available to an agent.  Alternatively, you might also check out the Zendesk Marketplace to see if an app solves for your specific pain points.
