    Hi. It looks like you need advanced features found in some of the upper plans. You can add custom ticket fields for the selection and then conditional fields (an app) to them show further checkbox custom fields for the questions.
    You make some of these fields mandatory to ensure customers fill them in.
    As long as your not on the most basic plan then I think this will all be available to you.
