Present ticket form only to certain end-users in the same organization?

Is it possible to only show a ticket form to a certain subgroup of end users, maybe based on a tag or user segment? 

For example, let's say that we have 500 end-users in our organization; however, there are 30 that I want to see a certain form that isn't available for the rest.  I don't want to give them full agent access, because other than this form, they should be treated the same as all other end-users.  Is there a way I can present only those 30 users with a specific form?

... or maybe based on the organization. We have a similar problem. We would need to display a different support form to a specific segment of users (based on a higher plan of our product) and do not want to necessarily have to create another brand just for this.
