How can I hide “Your email address field” in the form?

So once we hide the other forms and then what if we want to access the hide form for our internal access on admin side then how can we access them.

I'm receiving this error "Zendesk account linked to JIRA is no longer reachable" and this integration is established by admin only yet receiving this error.

is this error occurred due to wrong URL pasting from both side while set up integration?

What could be the reason?
