I have the same request as well, I would like the form selection function to be disabled in the request page. I'm using the Copenhagen v2 theme and it doesn't come with a document.ready function in the script. Tried adding the document.ready function and added the code you suggested, but doesn't work still (see image below).

Please help, thanks!
