Start a call with Talk based on a link in custom app

We have built a custom app that contains contact information to reach out to our suppliers. Below is a screenshot of how that looks for one of our suppliers. The functionality which we would like to build is that when an agent clicks on the phone icon then a call within Zendesk Talk is started. We try to make this work, though need some help. Is this possible? Thoughts?

I've touched base with a couple of folks throughout the talk team and it sounds like this isn't possible in a maintainable way. I'm going to leave this open in case someone from the community has a way to accomplish this.
