customized field / forms for end-users - Submit a request form?

Hi,

on each form, the fields Subject, Description, Attachments are mandatory [system default fields] ... is there any way to get rid of these?

I just want to create a basic simple form with only 3 customized fields ... how do I do that?

Hey walter maldeghem

Because these fields translate to Ticket Title and Comment, they are required.  That said, you can use a bit of code to hide and pre-fill them.  Details here: https://support.zendesk.com/hc/en-us/community/posts/5050937568666-Prefill-and-hide-Subject-Description-fields-of-specific-form-on-New-Request-Template

The comments on the support article also reference hiding the Attachments field, which is not mandatory, but helpful for end-users that need to supply additional context.  Hope this helps!
