Shopify and Zendesk Custom Forms

Hi,
We have Shopify and Zendesk integration for support. What we also want is that on our website we have contact us form and if someone submits it we want to create an entry in Zendesk. How we can achieve it?

Hi Pravin,
You can also embed the web widget on your own site, where the customer can fill out the ticket form that will create a ticket on your Zendesk account. You can check Using Web Widget (Classic) to embed customer service in your website, and Understanding and optimizing ticket forms for more info.

Thanks. But above links show adding form within chat, what I want is to add custom form under lets say contact us section of website and once user submits create zendesk ticket for it, attached is example of form what we want.

Hi Pravin,
You can also enable your Zendesk Help Center where customers can fill out forms that will create tickets on your account. Unfortunately, there is no native way to embed the ticket form directly to an external site, more information on how to present ticket forms to end-users can be found here.

but like some other users, I am also not able to open it, any thoughts?

Have you checked if your Help Center is activated? If not, you will not be able to access the link.

I have enabled it now. Still when I access above url I have few concerns, as I mentioned I just want to embed form within our shopify page, please see attached, here sections marked in red I dont want, so how to remove them before embedding inside the web page, we also dont want issue type selection which is present, can you tell me how to embed simple form with 2-3 fields and submit button?

When I try to copy javascript code(mainly html form element) and embed in website, I am getting 403 error, see this page where I embedded the form https://simmons-fine-jewelry-inc.myshopify.com/pages/about-us
Once its submitted its throwing 403. Any idea?

Hi Pravin,
Apologies for my previous response, you can only embed a custom form using API to an external site. Unfortunately, Zendesk does not allow iframing of Zendesk due to the inherent security risks involved in iframing a web application. As of the moment, if you would like your customers to submit requests from your external site, you should use the web widget or the custom form using our API
Regarding your ticket form, you need to create your custom ticket field and add it to the said form.
